An Indianapolis Healthcare company is seeking a Registered Nurse Case Manager that can perform the following duties:

Conducts initial member assessments, triages incoming telephone calls, responds to questions regarding medications and treatment instruction, records and reports member's conditions, provides education and teaching, and answers questions regarding diagnostic testing and treatment. Performs a variety of member communication activities in accordance with established standards and practices. Reviews clinical work done by LPN Case Manager.

ESSENTIAL FUNCTIONS:

- Confirms member acceptance into program

- Completes initial member assessments and establishes appropriate level of risk stratification utilizing sound clinical judgment and decision-making skills in performing triage

- Provides telephonic interventions for members assigned to the program

- Utilizes relevant data, such as medical records and member complaints, observations and listening skills, to identify and resolve problems

- Records, and maintains complete and accurate records of member communications regarding medical care within the electronic medical record system

- Communicates information verbally, as needed, and through complete and accurate documentation in the clinical record on a timely basis

- Maintains confidentiality in all aspects of the job

- Facilitates effective and efficient home visitation for high risk members by scheduling member's initial home visit with RN Field Case Manager

- Coordinates member care with other disciplines as needed and that are prescribed for assigned members

- Provides services to members that are culturally competent and linguistically appropriate

- May review clinical work done by LPN Case Manager.

- Effectively manages time to meet established productivity requirements and standards

- Exhibits exceptional customer service skills and personal service knowledge while meeting quality standards

- Effectively follows through on member requests and needs in a timely manner

- Seeks education/training or assistance with areas of concern

- Works effectively with minimal supervision as well works with fellow employees as a team

- Performs other related duties and responsibilities as deemed necessary

- The ability to communicate well verbally in a clear speaking voice with exemplary interpersonal communication skills and a professional demeanor are required along with excellent written communication skills

- Utilizes time off in an effective manner and provides adequate notice of time away from work

PHYSICAL DEMANDS:

Requires prolonged sitting and frequent use of the telephone and computer. Requires ability to distinguish letters and symbols and the use of computer terminal or personal computer. Requires normal vision, hearing and ability to communicate effectively both orally and written.

WORKING CONDITIONS:

The general environment is clean with comfortable temperature and moderate noise level.



Requirements:
EDUCATION AND EXPERIENCE REQUIREMENTS:

- Must be a graduate from accredited Registered Nurse program with current Indiana RN license.

- Minimum of one (1) year experience in an acute care setting or equivalent experience preferred.
