A Northwest Indianapolis company is seeking an individual for their Field RN Case Manager position.

GENERAL DESCRIPTION:

Responsibility of day-to-day management of the assigned members. Utilizes face-to-face and telephonic encounters with members to provide on-going self management support, referrals, and reminders for appropriate care, physician visits, and on-going assessments. Works with Primary Medical Providers of member to ensure proper management during and after the nurse care manager intervention.

ESSENTIAL FUNCTIONS:

- Confirms member acceptance into program

- Confirms collaboration between physician and member and obtains plan of care

- Facilitates effective and efficient home visitation for assigned case load

- Completes initial member assessments and risk stratifications utilizing sound clinical judgment and decision-making skills.

- Provides telephonic interventions for members assigned to the program

- Accurately records, and maintains complete and accurate records of member communications regarding medical care within the electronic medical record system

- Coordinates member care with other disciplines as needed and prescribed for the assigned members

- Communicates plans and changes to the physician and/or nursing supervisor and other staff through the care plan, written progress notes, and participation in care conferences.

- Participates in care conferences and other team meetings

- Communicate verbally in a clear speaking voice with exemplary interpersonal communication skills and a professional demeanor along with excellent written communication skills

- Maintains confidentiality in all aspects of the job

- Assists in the investigation of physician and member issues and complaints

- Demonstrates knowledge of resources available in the community, initiates referrals as appropriate, and sees that members are able to access needed resources and services

- Effectively follows through on member requests and needs in a timely manner

- Provides services to members that are culturally competent and linguistically appropriate.

- Effectively manages time to be able to meet established productivity requirements and standards

- May review clinical work done by LPN Case Manager

- Seeks education/training or assistance with areas of concern

- Licensed driver with automobile insurance in accordance with state requirements

- Works effectively with minimal supervision and works with fellow employees as a team

- Performs other related duties and responsibilities

- Utilizes time off in an effective manner and provides adequate notice of time away from work

WORKING CONDITIONS:

The general environment is clean with comfortable temperature and moderate noise level. Employees may work in a field environment with moderate noise levels, limited controlled temperature conditions, and may have potential exposure to strong odors, chemicals, equipment and instrument hazards, and medical hazardous wastes, physical substances and fumes. Tasks may require exposure to illness/disease, and work related stress and hostile individuals.



Requirements:
EDUCATION AND EXPERIENCE REQUIREMENTS:

- Graduate of an accredited school of Nursing with a current RN license to practice in the state employed

- Valid driver's license; proof of insurance

- Minimum of one (1) year of health care experience is preferred

PHYSICAL DEMANDS:

Requires stamina for long distance or all-day driving of a vehicle; Mobility to go into member's homes and the use of standard office equipment, including a computer; Vision to discern colors, assess medical situations, read printed materials and computer screen; hearing and speech to communicate in person and over the telephone; Perception required for making medical observations; Fieldwork requires standing and walking between work areas; Finger dexterity is needed to access, enter and retrieve data using a computer keyboard or calculator and to operate medical services equipment.
